Alleged: Back Injury

 Subject carried a bag of ice on his left shoulder and a shopping in his right hand as he briskly walked to his camper.  Subject alleges back injury as result of an on-the-job fall.

Alleged: Lumbar injury

Subject carried a large box of air conditioning equipment to his truck.  Subject alleges a lumbar injury on-the-job.  Subject was later observed working in the area.

Alleged:  Lower back, right knee injuries

Subject raking a yard.  Subject was doing yard work at several locations.  Subject alleges a lower and right knee injury from a fall in a local grocery store.

Alleged: Left elbow & shoulder, lower back

Subject loaded bags of groceries into her vehicle after shopping at a local store.

Alleged: back injury

Subject is loading cases of ceramic title inside of a hardware store.  Covert surveillance equipment was able to capture the subject's activities.   Once outside of the store, a store clerk loaded the title into the subject's truck as he stood there wearing his back brace.

Alleged: Back and neck injury

Subject who was claiming that she was unable to work due to her injuries was observed working inside of a local department store. 

Alleged: unable to get out, severe headaches, unable work due to injuries

Subject was observed shopping at local stores for hours.

Subject was observed riding his bike through the streets.

Subject carried a sheet of plywood into a house.  Subject was doing remodeling work on this house with a construction crew.

Subject was walking up stairs with a book pack on his back.

Subject carried a dog kennel from his house to his vehicle with a lab puppy inside.

Subject lifted a vacuum cleaner into the back of his truck.
